At Happy Ears Audiology, we believe that the cost of hearing care should never stand between a patient and the treatment they need. Our Affordable Care program is designed to help individuals in our community access the hearing healthcare services they deserve.

How Do I Apply?

  • Download or print the Affordable Care Application (PDF).
  • Complete the application in full.
  • If you were referred by another clinic, please include a copy of your hearing test with your application.
  • Send the completed application to our office by mail, fax, or email.

Fill out the form below and we'll be in touch within a few business days. You can attach copies of any relevant documents (hearing test results, etc.) directly here.

Accepted: images and PDFs. Max 10 MB per file. You can attach your hearing test results here if you have them.

Your information is submitted securely over an encrypted connection and handled in accordance with HIPAA guidelines.
